Being able to support a patient through a digital journey encourages them to take control of their health and allows for improved health outcomes and progress.   [Lifecard](https://www.lifecard.com/) is a personal health record dedicated to empowering patients to take control of their health. Lifecard allows patients to manage their health through a singular platform accessible through a mobile app and desktop to cater to patients anytime, anywhere, any place. Lifecard provides consumers with the opportunity to manage their health through a range of different features, including: - Tracking measurements (weight, blood glucose, etc.) - Sharing records with health professionals - Saving images and documents - Recording allergies - Recording immunisations - Tracking medications and prescriptions - Setting reminders - Tracking appointments Lifecard provides consumers with the ability to track not only their own health but their children’s and elderly relatives. Lifecard users can store information and communications securely for future reference. Being able to keep all relevant information in a singular place provides convenience and security to patients. The storage of this information reduces the need for paper records, is more convenient and allows ease of access. Lifecard is one of Global Health’s products aimed at digitally connecting clinicians to consumers. Lifecard is integrated with other Global Health products to improve a patient’s digital journey. The integration between Lifecard and [HotHealth](https://www.hothealth.com/) allows the configuration of forms to pre-fill information from the patient’s Lifecard record. This simple integration reduces data entry for the patient and leads to more accurate information capture for clinicians. Lifecard can log appointments for patients based on appointments listed in their clinicians [MasterCare+](https://www.mastercare.net.au/mastercare-plus/) calendar. From your MasterCare+ account, you are also able to securely send health documents, measurements and results directly from your clinical software to your patient’s Lifecard account. This secure message delivery process is facilitated by [ReferralNet](https://www.referralnet.com.au/), Global Health’s secure messaging provider. Similarly, any ReferralNet user can send information to a patient’s Lifecard from their own clinical or practice management system. Lifecard is especially helpful in the discharge stage of care. Users can update relevant measures and information for monitoring in the discharge process. 2021 hopes to see the integration of Apple Health with Lifecard to automatically populate relevant data from Apple Health into a patient’s Lifecard account for more convenient data capture. Lifecard is a great addition to a healthcare organisation’s digital offering. It promotes the inclusion of patients within their own care and empowers them to take control of their health.
